Chloe Nicole Schwartz, 4 days old, died Friday, April 20, 2012, at St. Marys Hospital in Rochester, Minn.

Chloe was born April 16, 2012, in Rochester to Alexandria Tabor of St. Peter, Minn. She is survived by her adoptive parents, Mark and Nicole Schwartz of Austin, and many other relatives and family friends.

Private family services are being planned for a later date, with Worlein Funeral Home of Austin assisting the family with arrangements.
