Salathiel Marc Forman, infant son of Marc and Alexis Forman, was stillborn Sunday, Nov. 4, 2012, at Mayo Clinic Health Systems-Austin.

Survivors include parents Marc and Alexis Forman and twin sister Mary Nakita Forman of Austin; paternal grandparents David (Sherri) Forman of Austin; maternal grandfather Luster (Keri) Green of Sacramento, Calif.; maternal grandmother Clare (Rodney Donis) Combes of Portland, Ore.; paternal great-grandfather Lester Forman of Omaha, Neb.; paternal great-grandmother Mary Kjerstad of Quinn, S.D.; maternal great-grandmothers Mary McGee of Portland and Ann Miller of Los Angeles, Calif.; aunts and uncles McKenzie Forman of Bismarck, N.D., Daniel Forman of Panama City, Fla., Brian Forman of Austin, Dominic Green, Damian Donis, Isaiah Donis, Elijah Donis and Monica Green, all of Portland, Brenden Green and Breauna Green, both of Sacramento. He was preceded in death by paternal great-grandmother Victoria Forman, paternal great-grandfather Konrad Kjerstad, maternal great-grandfathers Luster Green Sr. and George Combes.
